The present volume highlights progress of research in Madhumeha (Diabetes mellitus) during 20 century in India. Only significant achievements on pathophysiology of Madhumeha during 20º Century have been included.
Further newer concepts that have emerged during this period in the use of herbal drugs and mineral remedies in the treatment of Madhumeha have been emphasized.
However, the main aim of this volume is to prepare a document which would be a database referral text on herbal and mineral remedies developed in India vis-à-vis Madhumeha.
This volume could be prepared through extensive survey of literature carried out at selected institutions for Ayurvedic and Siddha Medicine and through personal contacts with peers in these fields. Their help is sincerely acknowledged.

Madhumeha (Diabetes mellitus) is a type of Prameha which is characterised by discharge of sweet urine through urethera. The history of origin of Prameha can be traced to an incident of disruption of 'Yayga' conducted by Daksha Prajapati. According to Caraka, Prameha resulted from over eating of 'Havish' offered in the 'Yagya' Tridosha regarded as the functional basis of body mechanism control the state of health of an individual. Vata, pitta and kapha, the functional tri-pod of the body maintain the functional stability of the body.
Individual dosha maintains an equilibrum amongst themselves in a healthy state. Vitiation of this equilibrium will induce a state of ill health which may proceed up to the stage of disease.
Madhumeha in Ayurveda consists of a three clinical types indentified as Vataja, Pittaja and Kaphaja. There are some 20 sub types of Prameha which can also be indentified (Chandola and Tripathi).
Kaphaja Madhumeha may be related to high growth homone in the system and pittaja is the one which may be related to high level of glucocorticoids. In the progression of diabetes, the initial stage of Prameha, the Kapha dosha is at high level which changes into Kaph Kashay later on. Similarly Pittaja Prameha leads to Pitta Kashya. However, there is a common denomi-nator of Vata Briddhi in both Kaphaja and Pittaja Prameha which in the terminal state lead to a stage of Madhumeha, sub 1 type of Vataja Prameha.
It has been hypothesied that 20 types of Prameha are different types of Madhumeha. Vataja, Pittaja and Kaphaja Prameha can be correlated with different stages of Madhumeha during its progression.
