Introduction
Ayurveda is a well-known science of life, because of its three principle objectives; Swasthasya Swasthya Rakshanam (To prolong life and promote perfect health), Aturasya Vikara (To completely know the disease) and Vikara Prashamana (To completely eradicate the disease). Roganidana is a unique subjective and objective diagnostic oriented department of Ayurveda mainly deals about different diagnostic parameters to surmount the Aturasya vikara by means Roga and Rogi pareeksha (Examination of Patient and Disease).Among them Nidana Panchaka, it is evolved with a combination of two words pancha means 'five' and nidana means 'methods of diagnosis' i.e. five means of diagnosis and it is considered as unique diagnostic tool. Namely Nidana (Causative factor), Poorvaroopa (Premonitory clinical features), Roopa (Clinical features), Upashaya (Confirmatory diagnostic therapy/ Explorative therapy) and Samprapti (Pathogenesis/Disease Process).
(च.सु. 20/20) Disease is considered to be king because clinically it manifest with multiple dimensions, some features before manifestation of the disease called poorvaroopa, some after the manifestation of the disease called roopa and some during chronic or complicated stage of the disease known as upadrava, so to overcome the complete knowledge of the disease one has to examine the disease thoroughly by means of Nidana Panchaka and later initiate the treatment for the betterment of the patient and to get success in the society.
न हि सर्वविकारणा नामतोडस्ति ध्रुवा स्थिति : ! (च. सु. 18/44) As mentioned that, it is not possible to give name for all the diseases, whatever is needed, is to know the nature of the pathology and the nature of the patient is enough to know the nature of the disease. Thereafter, one should plan the treatment by applying the knowledge of disease gained by the Nidana Panchaka. A physician intiated the treatment without proper diagnosis or analysing the factors (in case of unknown disease) of the disease can accomplish the desired objects only by the chance, even he is well acquainted with knowledge of chikitsa. On the other hand a physician, who is admostered treatment after the accurate diagnosis or critical analysis of the factors of disease, is sure to accomplish the desired object without doubt.
Thus, all the ancient treatises of ayurveda has emphasized to follow specific diagnostic methods for reaching the accurate diagnosis of the disease prior to their treatment. The treatises like Charaka, Madhava. Sushruta etc have described various rogapareeksha in terms of unique diagnostic tool called Nidana Panchaka in specific way. So in this book an initiation is taking to explain the concept of Nidana Panchaka in the clinical point of view to know the diagnostic aspect.
