Preface
The incidence of colorectal diseases are progressively increasing, with the changes in the diet and life style. The anorectal diseases are ignored and neglected on behalf of the patients and physicians both, so sometimes the disease becomes incurable, life threatening or fatal. The operative results of anorectal diseases are not always satisfactory, even by skilled hand. Complications like rectal incontinence and recurrences are common problems after radical surgery, which disturbs the quality of life. The introduction of kshar sutra technique in the management of fistula in and by professor P.J. Deshpande, Banaras Hindu University, Varanasi, in the decade of 60th and 70th revolutionized in the field of Anorectal surgery. Now a days anorectal diseases and kshar sutra surgery is emerging as a superspeciality branch in the field of surgery. It is widely practiced all over in India and in some foreign countries.
The present book is aimed, not only to fulfill the need of undergraduates and postgraduates scholars, but also to the practitioners, academicians and researchers, actively engaged in the practice of kshar therapy and colorectal diseases. This book not only covers the anorectal diseases, but a brief accounts on complete coloproctological disorders, because in most of the anorectal diseases, large bowel is the primary cause of the diseases like colitis, polypoid diseases, malignant diseases, rectal prolapse, Crohn's disease etc. Anatomical and physiological details mentioned in the book will definitely help to the scholars in the diagnosis and management of colorectal diseases. The original photographs and figures will definitely save the time of readers in going deep in the literature and will help in the better understanding of the techniques of Kshar therapy. The figures of original fistulogram will definitely disclose the mystry of anal fistula and its relation to pararectal spaces, anorectal canal and adjacent pelvic organs.
The operative "results of kshar sutra treatment in anorectal diseases like anal fistula, piles, rectal polyp, anal fissure, pilonidal sinus, and rectal prolapse, is given in the end of concerned chapter, which are based on clinical datas collected during treatment period. Mersilk has been used in the management of rectal prolapse with good results. Role of kshar sutra in the management of chronic anal fissure will definitely replace the modem surgical techniques, the lateral and posterior sphincterotomy in comming days. The posterior sphincterotomy by kshar sutra has been widely elaborated in this book with excellent clinical results.
An attempt has been made to review the complete modem literature on colorectal diseases and scattered literature on guda roga (Anorectal diseases) as discribed in various ancient text books in Indian medicine.
Though Colorectal surgery is very challenging with unsatisfactory postoperative results, still we hope this book will be boon to the medical students, teachers and research workers practicing and contibuting the role of Kshar Chikitsa and Anorectal diseases in different parts of the country and abroad. Nothing could provide us imminence pleasure than intelligent criticism with creative opinions and suggestions from eminent scholars actively engaged in the Kshar Chikitsa and colorectal practice, which will be improved in the next edition.
